First of all, you have to insert an 'Ad' in the popular newspapers that you wish to change the name.

For example:

"I, Mukesh PANDIT state that hereafter (with effect from 1st Nov 2010 / with immediate effect) I wish to be known as Mukesh PRAJAPATI, and for all intents and purposes this change is permanent."

Then go the court and given the same statement before an Oath Commissioner, or an Authorised Notary of the court. Then you can start calling yourself by the new name.

In the passport and Election card, with the stamped court paper you have to apply again for new documents. This is a rather cumbersome process.

so for that u have to come to Bihar n go to Patna notary then purchase a Non judicial stamp of re. 50or100or related price then make an affidavit about ur name changing. then publish it at least 2 newspaper for make urself known by all. then contact to related officials like passport office n all. but as far as my knowledge is concern Voter id card is not been reissued with changes. so for more go to ADM at collecteriate.
